What Does an ENT Look At During a Nasal Endoscopy?

One of the most useful tools an ENT specialist uses to diagnose sinus and nasal problems is a nasal endoscopy.

Many patients are surprised at how quick and helpful this procedure is. A nasal endoscope allows your ENT doctor to see areas deep inside the nose and sinuses that simply cannot be evaluated well with a regular flashlight exam.

At SCENT Houston, Dr. Madeleine S. Herman frequently performs nasal endoscopy to evaluate nasal congestion, sinus infections, nasal obstruction, allergies, postnasal drip, loss of smell, and more.

What Is a Nasal Endoscopy?

A nasal endoscopy is a procedure where a thin camera called an endoscope is gently inserted into the nose to examine the nasal passages and sinus openings.

The procedure:

  • Is performed in the office

  • Usually takes only a few minutes

  • Does not require sedation

  • Is generally very well tolerated

Most patients describe it as mildly uncomfortable at most.

Why Would an ENT Perform a Nasal Endoscopy?

Nasal endoscopy helps diagnose many common ENT conditions, including:

  • Chronic sinus infections

  • Nasal obstruction

  • Deviated septum

  • Nasal polyps

  • Allergies

  • Postnasal drip

  • Nosebleeds

  • Loss of smell

  • Chronic congestion

  • Sinus headaches

It allows your ENT physician to get a much more detailed view than a standard exam.

What Does an ENT Look For During a Nasal Endoscopy?

1. Deviated Septum

The septum is the wall dividing the two sides of the nose. If it is crooked or deviated, it can block airflow and contribute to congestion, snoring, and sinus problems.

An ENT evaluates:

  • Severity of the deviation

  • Areas of blockage

  • Impact on breathing and sinus drainage

2. Nasal Polyps

Nasal polyps are soft inflammatory growths that can block airflow and sinus drainage.

During endoscopy, your ENT looks for:

  • Presence of polyps

  • Size and location

  • Signs of chronic inflammation

Polyps are commonly associated with chronic sinusitis and allergies.

3. Signs of Infection or Inflammation

An ENT examines the nose and sinus drainage pathways for:

  • Swelling

  • Thick mucus

  • Pus or infection

  • Redness and inflammation

These findings may suggest chronic sinusitis or active infection.

4. Turbinate Enlargement

Turbinates are structures inside the nose that help humidify and filter air.

When enlarged, they may contribute to:

  • Chronic congestion

  • Difficulty breathing through the nose

  • Snoring

Your ENT evaluates the size of the turbinates and their contribution to blockage.

5. Sinus Drainage Pathways

The sinus openings are carefully examined to look for:

  • Blockage

  • Swelling

  • Polyps

  • Poor drainage

This is especially important in patients with recurrent sinus infections or chronic sinus pressure.

6. Sources of Nosebleeds

In patients with recurrent nosebleeds, nasal endoscopy helps identify:

  • Irritated blood vessels

  • Areas of dryness

  • Crusting

  • Less commonly, abnormal growths

7. Signs of Allergies

Allergic inflammation inside the nose may appear:

  • Pale

  • Swollen

  • Boggy

  • Mucus-filled

Nasal endoscopy can help determine whether allergies may be contributing to symptoms.

8. Masses or Abnormal Growths

Although uncommon, ENT specialists also evaluate for:

  • Tumors

  • Unusual masses

  • Structural abnormalities

Persistent one-sided blockage or bleeding may require further evaluation.

Is Nasal Endoscopy Painful?

Most patients tolerate nasal endoscopy very well.

A topical numbing spray and/or decongestant may be used beforehand to improve comfort.

Patients often say:

  • “That was easier than I expected.”

  • “It just felt a little strange.”

  • “It wasn’t painful.”

When Should You Have a Nasal Endoscopy?

Your ENT may recommend nasal endoscopy if you have:

  • Chronic nasal congestion

  • Recurrent sinus infections

  • Difficulty breathing through your nose

  • Loss of smell

  • Postnasal drip

  • Persistent sinus pressure

  • Nosebleeds

  • Snoring or sleep issues
