When Should You Get a Tonsillectomy? A Complete Guide for Kids and Adults

What Is a Tonsillectomy?
A tonsillectomy is a surgical procedure to remove the tonsils — the two lymphoid tissues located at the back of the throat. Tonsils help fight infection in early childhood, but in many patients they become more harmful than helpful due to repeated infections, airway obstruction, or chronic inflammation.
Tonsillectomy is one of the most common ENT surgeries performed in both children and adults and can significantly improve quality of life when appropriately indicated.
1. Recurrent Tonsillitis or Strep Throat
One of the most common reasons for tonsil removal is frequent throat infections, especially bacterial tonsillitis or strep throat.
You may be a candidate for tonsillectomy if you experience:
7 or more throat infections in one year, or
5 or more infections per year for two consecutive years, or
3 or more infections per year for three years in a row
Symptoms typically include severe sore throat, fever, swollen lymph nodes, white patches on the tonsils, missed school or work, and repeated antibiotic use.
If infections are interfering with daily life, work, school attendance, or overall health, surgery may be recommended.
2. Enlarged Tonsils Causing Breathing or Sleep Problems
Large tonsils can partially block the airway, especially in children but also in adults.
Signs that enlarged tonsils may require removal include:
Loud snoring
Pauses in breathing during sleep (sleep apnea)
Restless sleep or mouth breathing
Daytime fatigue or behavioral issues in children
Difficulty swallowing solid foods
In children, tonsillectomy (often combined with adenoidectomy) is one of the most effective treatments for obstructive sleep apnea.
Adults with airway obstruction from enlarged tonsils may also benefit significantly from tonsil removal.
3. Chronic Tonsil Stones (Tonsilloliths) and Bad Breath
Tonsil stones form when debris becomes trapped in the tonsil crypts and hardens. They can cause:
Persistent bad breath
Throat irritation
A foreign body sensation
Recurrent infections
Visible white debris in the tonsils
If tonsil stones are frequent, difficult to remove, or causing significant quality-of-life issues despite good oral hygiene and irrigation, tonsillectomy may be the most effective long-term solution.
4. Chronic Sore Throat or Tonsil Inflammation
Some patients experience constant throat discomfort, enlarged tonsils, or chronic inflammation even without frequent infections. Symptoms may include:
Ongoing throat pain
Difficulty swallowing
Voice changes
Recurrent swelling
Persistent irritation
When medical treatment fails to control symptoms, tonsillectomy may provide relief.
5. Suspicious Tonsil Growth or Asymmetry
Unilateral tonsil enlargement, especially when associated with pain, bleeding, weight loss, or neck masses, requires prompt evaluation. While most cases are benign, ENT specialists sometimes recommend tonsil removal to rule out serious conditions.

What Age Is Best for Tonsillectomy?
Tonsillectomy can be safely performed in both children and adults. Children often recover faster, but adults can still experience excellent outcomes when surgery is appropriately indicated. Recovery typically takes:
Children: 7–10 days
Adults: 10–14 days
Your ENT will discuss pain management, hydration, activity restrictions, and recovery expectations.

Frequent throat infections
